Job summary

North Florida Rehabilitation Hospital is seeking a Physical Therapist for day shift, full-time employment. You will evaluate patients, create and implement individualized treatment plans, and work with staff and families to support ongoing recovery.

The role requires an accredited Physical Therapy degree, active licensure in the state, and CPR certification, with a focus on patient-centered care and regulatory compliance.

Qualifications

• Degree from an accredited Physical Therapy program.
• Current and unrestricted Physical Therapy license in the state where services are rendered.
• Current CPR certification.

Responsibilities

• Evaluate a patient’s condition and develop a treatment plan; guide progress day by day.
• Instruct nursing staff and families on follow-through programs that build on progress.
• Communicate patient progress to supervisor and team; assist with scheduling and post charges.
• Document patient care in accordance with regulatory and accrediting requirements.
• Maintain equipment and work area in a safe, clean condition; comply with professional standards and state/federal laws.
• Perform other duties as assigned.
