Job summary

The Quality Center for Rehabilitation and Healing is seeking an Experienced Physical Therapist (PT) for our facility. Daily pay is now offered along with competitive compensation.

You will assess referred patients, develop treatment plans, document progress, and educate families. The role requires supervising PTAs and collaborating with multidisciplinary teams to deliver patient-centered care in a long-term care setting.

Qualifications

• Graduated from an accredited Physical Therapy college or university.
• Must hold a valid professional license and registration for state of practice.
• Experience supervising Physical Therapy Assistants.
• Experience with long-term care and/or geriatric care preferred.
• Strong documentation skills and record keeping.
• Experience with evaluations, weekly progress notes, recertification and discharge documentation.
• Collaborates with all disciplines to provide patient-centered care.
• Excellent patient service and clinical excellence.
• Punctual, professional, and adheres to scheduled hours.
• Performs duties as assigned by Director of Rehabilitation.
• Confidentiality and HIPAA compliance; knowledge of Residents' Rights.

Responsibilities

• Assess referred patients to determine rehabilitation needs and start direct patient care.
• Develop and administer treatment programs with goals based on patient capacity and tolerance.
• Document and record patient condition; educate patients and families in therapy methods.
• Set realistic, achievable goals and monitor progress.
• Oversee and supervise Physical Therapy Assistants as required.
• Coordinate with other disciplines to ensure comprehensive patient-centered care.
