Reliant Rehabilitation is seeking a licensed Physical Therapist to join our therapy team serving skilled nursing facilities. This role offers part-time or full-time schedules and the chance to impact patient outcomes through evidence-based care and clinical leadership development.

Responsibilities include evaluation, treatment planning, supervising rehab techs and assistants, and collaboration with facility staff to optimize patient function while adhering to state and federal guidelines.

Qualifications

• Graduate of an APTA-accredited school of physical therapy.
• Must hold a valid state physical therapist license.
• Proficient with electronic medical records and clinical documentation.

Responsibilities

• Provide physical therapy evaluation and plan of care for patients.
• Develop and update treatment plans and discharge plans.
• Supervise PTAs and Rehab Techs during group and individual therapy.
• Collaborate with facility staff to optimize patient outcomes.
• Maintain compliance with state and federal regulations and infection control.
