Physical Therapist – CFMH Rehab (Open)
Responsibilities:
- Confirm and interpret physician referrals, perform patient assessments, and plan and implement individualized treatment programs taking into account age and medical diagnoses in accordance with regulations.
- Provide patient care based on physician referral and age-appropriate assessment, planning and implementing treatment programs to achieve patient and family goals.
- Coordinate patient care and clinical activities through written and/or verbal communication with patients, families, therapists, Clinical Team Leaders, managers, physicians, and other healthcare team members.
- Document care in the patient’s medical record and complete other patient care–related forms in compliance with regulatory agencies and department/hospital policies and procedures.
- Supervise PTAs and collaborate with other therapy staff, support staff, volunteers, and students to achieve efficient clinical activity and quality patient care.
- Participate in other patient care and departmental/professional activities as requested or assigned, including presenting/attending in-services, performance improvement, program development/evaluation, new employee orientation, committee membership, peer review and attending staff meetings.
- Supervise Physical Therapist Assistants (PTAs) according to State of Virginia licensure and Federal regulations.
- Orient new employees and may provide guidance to volunteers, students, support, or contract staff.
Qualifications:
- Graduate of an approved Physical Therapy program.
- Bachelor’s degree in Physical Therapy with 5 years of experience, or a Master’s degree or Doctor of Physical Therapy (DPT).
- Licensed to practice in the Commonwealth of Virginia by the Virginia Board of Physical Therapy.
- Effective interpersonal, communication, customer service, and teamwork skills.
