Physical Therapist Case Manager: Home Health

Position Details

This is a full-time, field-based role requiring you to visit patients' homes. You will evaluate and treat patients, develop care plans, and coordinate with physicians, aiming to restore function and promote independence.

Responsibilities

1. Perform evaluations and develop care plans in patients' homes.
2. Assist patients with mobility and independence.
3. Create detailed treatment plans.
4. Coordinate care with physicians.
5. Report patient progress to MDs.
