Schedule Details/Additional Information:

Typically Monday through Friday, 8am to 4:30pm. Weekend rotation with one weekend every 8 to 12 weeks after the first 6 months in role. No holiday or night shifts required. Area of responsibility includes Mequon, Thiensville, Glendale, River Hills, Brown Deer, Bayshore, Bayside, Fox Point, Whitefish Bay, and some zip codes on Milwaukee's north side. Minimal travel outside these areas.

Major Responsibilities:

  • Conduct physical therapy evaluations, establish care and treatment goals, interpret physician referrals, and perform musculoskeletal, neuromuscular, cardiopulmonary, and integument evaluations. Evaluate outcomes within appropriate timeframes.
  • Provide ongoing assessment of patient responses, teach and apply interventions, update and revise care plans, and report to physicians regularly.
  • Communicate issues to physicians and management, document care and patient records per regulations.
  • Manage caseload including scheduling, coverage needs, and patient medical conditions. Advocate for patients to ensure optimal outcomes. Review caseload for compliance and efficiency metrics.
  • Potentially supervise Physical Therapist Assistants, ensuring compliance with care plans and state regulations.
  • Participate in joint visits with Home Health Aides, evaluate patients, and develop plans for adaptive devices. Educate patients and caregivers.
  • Discuss caseload and census issues with management. Communicate effectively with physicians and team members.
  • Participate in audits, quality improvement, discharge planning, and utilize community resources for continuity of care.
  • Serve as a resource for staff training, mentoring, and student supervision.
  • Perform patient positioning, transferring duties, and other physical tasks, ensuring proper technique.
  • Provide age-appropriate care, understanding growth, development, and patient-specific needs.

Licensure/Certification:

  • Physical Therapy License in Illinois or Wisconsin.
  • Basic Life Support (BLS) certification from AHA.
  • Valid driver’s license.

Education/Experience:

  • Doctorate in Physical Therapy or equivalent, with ongoing continuing education.
  • At least 1 year of clinical experience or strong clinical affiliation experience.

Knowledge, Skills & Abilities:

  • Knowledge of modalities and exercise indications and contraindications.
  • Skill in using modalities, exercise equipment, and devices.
  • Proficiency in clinical skills, independent judgment, good human relations, organization, problem-solving, and decision-making skills.
  • Basic proficiency in Microsoft Office.

Physical Requirements and Working Conditions:

  • Transportation to attend home visits.
  • Manual dexterity for medical equipment and documentation.
  • Physical ability to move in confined spaces, lift up to 50 lbs, and perform transfers.
  • Ability to sit, stand, walk, climb stairs, and sustain patient care activities for extended periods.
