Responsibilities:

As a Physical Therapist Case Manager, you will be responsible for:

1. Assessing patient needs and developing treatment plans.
2. Providing physical therapy services in accordance with the treatment plan.
3. Collaborating with other healthcare professionals to ensure comprehensive patient care.
4. Monitoring patient progress and adjusting treatment plans as necessary.
5. Educating patients and their families about physical therapy and rehabilitation.

Minimum Requirements:

To qualify for this position, you must have:

1. A valid Physical Therapist license.
2. Experience in home health care.
3. Strong communication and interpersonal skills.
4. The ability to work independently and manage a caseload.
