Job summary

PARADIGM REHAB & HEALTH CARE LLC in Keller, Texas is looking for a Physical Therapist Assistant PRN to implement patient care plans and communicate with the healthcare team. You will educate patients and their families on safety, work independently, and assess when to seek support.

The ideal candidate will have an Associate Degree in Physical Therapy, current BSL/CPR certification, and home health experience. Knowledge of WellSky EMR software is a plus.

Qualifications

• Must have an Associate Degree in Physical Therapy and current BSL/CPR certification.
• Home health experience is required.
• Valid Texas Driver's License and automobile insurance are necessary.

Responsibilities

Responsibilities and Duties

• Implement plan of care in consultation with physical therapist and other care‑team members.
• Communicate findings or changes in regard to patients back to the physical therapist and other care‑team members.
• Educate patients and their families in basic home safety and medical equipment safety.
• Solution driven, self‑motivated, and inspiring to patients to reach their goals.
• Work independently without direct oversight.
• Able to discern when to call for support and communicate challenges.
• Familiar and comfortable with technology.

Qualifications and Skills
