As a PTA in this role, you will become an integral teammate in our innovative Take Your Therapist Home Program.

What is Take Your Therapist Home?

In a world of shorter and shorter SNF stays, TYTH is a one‑of‑a‑kind program where you, as the clinician, work directly with patients in the skilled nursing facility (SNF) and home health, providing a continuity of care that allows you to have a greater impact on patient outcomes.

You will be primarily in the home health setting, with dedicated time and caseload support for the SNF. You will have a localized coverage area in the surrounding area of the SNF—no ever‑expanding home care coverage areas. Your schedule is based on a traditional work week.

Facility Location

Shelby – 46100 Schoenherr Road, Shelby Township, MI 48315. This hybrid role provides Home Health therapy services in conjunction with SNF‑based therapy services. Home Health geographic area is the vicinity around our Shelby Township facility. Home care services are paid per visit, and SNF services are paid at an hourly rate.

Schedule

• Full Time – Monday to Friday.

Required Qualifications

• Successful completion of an accredited Physical Therapy Assistant program.
• Licensure as required by the State of Michigan.

Skills and Abilities

• Ability to exercise professional judgement and initiative when analyzing problems and recommending solutions.
• Initiative and flexibility in the work environment.
• Ability to react effectively to the needs of patients and their families.
• Physical ability to bend, position, transfer and lift patients weighing from 80 pounds to over 300 pounds, without restriction, utilizing lifting devices clinically recommended for the patient.
