Job summary

Responsibilities

• Provide support care of residents who seek short-term/transitional care with the goal to recover and rehabilitate under supervision of licensed therapists.
• Assist in providing appropriate aspects of quality patient care, under supervision of licensed therapists.
• Document all services provided to ensure accurate and timely billing of therapy services.
• Ensure patient care follows the Plan of Care, organization guidelines, and professional standards per Federal/State guidelines.
• Under PT supervision, develop and implement individualized therapy programs to restore and enhance performance.
• Identify need for referrals to other disciplines to address comprehensive patient needs under PT supervision.
• Other duties may be assigned as needed.
