Job summary

You will document care, follow the Plan of Care, and coordinate with the rehab team to refer to other disciplines when needed.

Qualifications

• High School Diploma required.
• Requires valid Physical Therapy Assistant license or certification in the state of practice, or proof of license eligibility.

Responsibilities

• Provide support care to residents seeking short-term rehabilitation.
• Assist in providing quality patient care under supervision of licensed therapists.
• Document all services to ensure accurate billing.
• Assures that patient care follows the Plan of Care and professional standards.
• Under supervision of a Physical Therapist, develops comprehensive plans of care and implements individualized therapy programs.
• Identify need for and refer to other disciplines to address patient needs under supervision of a Physical Therapist.
• Other duties may be assigned.
