Concept Rehab is seeking a PRN Physical Therapist Assistant (PTA) to provide skilled, patient-centered care in a post-acute setting in Charlevoix, Michigan.

Under the direction of a licensed Physical Therapist, you will assist with mobility and functional activities, monitor progress, and contribute to a supportive interdisciplinary team. Flexible PRN scheduling and opportunities for CEUs are offered to help you grow professionally.

Qualifications

• Graduate of an accredited Physical Therapist Assistant (PTA) program.
• Current Michigan PTA license in good standing.
• Strong communication and organizational skills.
• Dependable and professional.
• Ability to work independently and as part of a team.
• Previous skilled nursing/post-acute experience is a plus, but not required.

Responsibilities

• Provide skilled treatment according to individualized care plans.
• Help patients improve mobility, strength, balance, transfers, and functional independence.
• Assist patients with therapeutic exercises and functional activities.
• Monitor patient response and progress.
• Communicate effectively with the supervising PT and interdisciplinary team.
• Complete accurate and timely documentation.
• Encourage and motivate patients throughout their rehabilitation journey.
• Provide compassionate, patient-centered care.
