Job summary

Concept Rehab is seeking a PRN Occupational Therapist to join our therapy team in Charlevoix, MI. The role offers flexible scheduling and a supportive clinical leadership within a therapist-owned practice.

As a PRN OT, you will evaluate patients, develop individualized plans, and provide skilled services aimed at improving independence in daily activities, while collaborating with nursing and medical staff to ensure quality patient care.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current Michigan Occupational Therapist license.
• Strong clinical, communication, and organizational skills.
• Ability to work independently while collaborating with a team.
• Skilled nursing or post-acute rehab experience is preferred, but not required.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide skilled occupational therapy services based on patient needs and goals.
• Help patients improve independence with activities of daily living.
• Address strength, coordination, cognition, positioning, mobility, and functional skills.
• Monitor progress and modify treatment plans as appropriate.
• Complete timely and accurate clinical documentation.
• Collaborate with nursing, therapy, physicians, patients, families, and facility staff.
• Participate in care planning and discharge planning as needed.
