Job summary

Pleasant Hill Post Acute, operated by PACS Group, is seeking a compassionate Physical Therapist Assistant (PTA) to join our rehabilitation team on a PRN basis in Pleasant Hill, CA. The PTA works under the supervision of a Physical Therapist to provide skilled treatments that promote mobility, independence, safety, and quality of life for residents.

Responsibilities include documenting care, collaborating with the interdisciplinary team, and educating residents and families.

Qualifications

• Associates degree from an accredited PTA program required.
• Current California PTA license required.
• Skilled nursing or post-acute rehab experience preferred.
• Strong communication, organization, and teamwork skills.
• Dependable and flexible with PRN/as-needed schedule.

Responsibilities

• Provide skilled physical therapy treatments and interventions as directed by the supervising Physical Therapist.
• Assist residents in achieving individualized mobility and functional goals.
• Complete timely, accurate, and compliant clinical documentation.
• Maintain a safe and clean treatment environment.
• Communicate progress with the supervising PT and participate in discharge planning.
• Collaborate with nursing and other interdisciplinary team members.
• Participate in Resident Care Conferences and rehab meetings.
• Educate residents, families, and caregivers regarding treatment plans and home exercise programs.
• Assist with recommendations for durable medical equipment and adaptive devices.
• Provide staff education and in-service training as needed.
• Report equipment concerns and help ensure rehabilitation equipment remains safe and functional.
• Maintain confidentiality of resident and company information.
• Develop positive relationships with residents, families, coworkers, and healthcare professionals.

Position Type: PRN / As Needed

Pay: $50–$53 per hour, depending on experience

Join Our Rehabilitation Team Pleasant Hill Post Acute is seeking a compassionate, motivated, and dependable Physical Therapist Assistant (PTA) to join our rehabilitation team on a PRN basis.

The PTA works under the direction of the supervising Physical Therapist to provide high-quality rehabilitation services that promote mobility, independence, safety, and overall quality of life for our residents.

Provide skilled physical therapy treatments and interventions as directed by the supervising Physical Therapist.

Assist residents in achieving individualized mobility and functional goals.

Complete timely, accurate, and compliant clinical documentation.

Maintain a safe and clean treatment environment while following infection control and safety protocols.

Communicate regularly with the supervising Physical Therapist regarding resident progress, goals, changes in condition, discharge planning, and care coordination.

Collaborate with nursing and other members of the interdisciplinary team.

Participate in Resident Care Conferences and rehabilitation meetings as needed.

Assist with training and support of Restorative Aides.

Educate residents, families, and caregivers regarding treatment plans, home exercise programs, safety techniques, fall prevention, and maintenance programs.

Assist with recommendations for durable medical equipment and adaptive equipment to promote resident safety and independence.

Provide staff education and in-service training as needed.

Report equipment concerns and help ensure rehabilitation equipment remains safe and functional.

Participate in quality assurance and performance improvement activities.

Maintain strict confidentiality of resident and company information.

Develop positive and professional relationships with residents, families, coworkers, and healthcare professionals.

Associate Degree from an accredited Physical Therapist Assistant (PTA) program required.

Current California Physical Therapist Assistant license required.

Skilled nursing or post-acute rehabilitation experience preferred.
