Job summary

Synergy Care, Inc. is seeking a qualified Licensed Physical Therapist Assistant for a PRN position at an Independent Living Community in Peoria, IL. The PTA will provide quality patient care, perform evaluations, and contribute to treatment planning and discharge processes.

Qualifications

• Must be a graduate of an accredited school for Physical Therapist Assistant or will graduate within six months.
• Current license as a Physical Therapist Assistant in Illinois or eligibility to obtain.
• PRN/part-time availability and ability to work in an Independent Living Community.

Responsibilities

Job responsibilities:

Provides physical therapy and related services for patients, caregivers and facilities in accordance with the principles and practices of occupational therapy and with Synergy Management Solutions guidelines. Provides quality PT including: Evaluation of patients, development of initial and ongoing treatment & discharge plan, supervision of assistants and aides & participates in case management. Represents Synergy Management Solutions’ commitment to quality & service in all interactions with facility. Adheres to all state regulations regarding occupational therapy.
In order to be considered for this position you must be a graduate of an accredited school for Physical Therapist Assistant or will graduate from such an institution within the next six (6) months.
