Location: Keystone Pointe, Lagrange, Ohio (Five Star Facility), approximately 10 minutes from Elyria.

Position Type: PRN (as needed) with flexible scheduling, including weekdays, weekends, staff vacations, and holidays.

Qualifications

• Ohio PTA license required
• Associate degree in Physical Therapy Assistance
• Experience in Skilled Nursing and familiarity with Rehab Optima/Net Health preferred
