Job summary

Piedmont Hills Center for Nursing & Rehabilitation is seeking a Physical Therapist Assistant (PTA) for a flexible PRN role in a skilled nursing and long-term care setting. You will work under a licensed Physical Therapist and join an interdisciplinary care team to help residents regain strength and independence.

The position offers flexible PRN hours with day shifts preferred, travel to nearby affiliated facilities as needed, and opportunities for professional development within a supportive

Qualifications

• Graduate of an accredited PTA program.
• Current state licensure as a PTA.
• Experience in skilled nursing or rehab preferred but not required.
• Ability to assist residents with transfers and mobility up to 50 lb.
• Strong communication, organizational and teamwork skills.
• Compassionate, resident-centered approach to care.

Responsibilities

• Provide skilled physical therapy treatments under licensed PT supervision.
• Assist residents in improving mobility, balance, strength, endurance and independence.
• Document treatment sessions, progress notes and therapy daily per guidelines.
• Monitor resident response to treatment and communicate progress to the PT.
• Assist with discharge planning and home exercise education for residents and families.
• Collaborate with nursing and care teams to optimize resident outcomes.
• Participate in care conferences and quality improvement activities.
• Travel to nearby affiliated facilities to support resident care.

As a Physical Therapist Assistant, you'll work alongside an experienced Physical Therapist and interdisciplinary care team to provide compassionate, high-quality rehabilitation services that improve the lives of our residents.

Position Details

• Position: Physical Therapist Assistant (PTA)

Key Responsibilities

• Provide skilled physical therapy treatments under the direction and supervision of a licensed Physical Therapist.




• Assist residents in improving mobility, balance, strength, endurance, and overall functional independence.




• Accurately document treatment sessions, resident progress, and daily therapy notes in accordance with regulatory and facility guidelines.




• Monitor resident response to treatment and communicate progress or concerns with the supervising Physical Therapist.




• Assist with discharge planning and educate residents and families on home exercise programs and safety techniques.




• Collaborate with nursing staff, physicians, and the interdisciplinary care team to achieve the best possible resident outcomes.




• Participate in care conferences, departmental meetings, and quality improvement initiatives.




• Travel to nearby affiliated facilities as needed to support resident care.

Qualifications

• Graduate of an accredited Physical Therapist Assistant (PTA) program.




• Current state licensure as a Physical Therapist Assistant.




• Previous experience in a skilled nursing, rehabilitation, or long-term care setting is preferred but not required.




• Ability to safely assist residents with transfers and mobility, including lifting up to 50 pounds when necessary.




• Strong communication, organizational, and teamwork skills.
