Job summary

Sweetwater Care in Maxwell, CA is seeking a licensed Physical Therapist Assistant (PTA) to support patients recovering from serious illness, injury or medical events. Under the supervision of a Physical Therapist, you help restore mobility, balance and function, and work to regain independence.

Responsibilities include providing care to residents, documenting services for billing, ensuring Plan of Care compliance, and coordinating with other disciplines as needed.

Qualifications

• Requires valid Physical Therapy Assistant license or certification in the state of practice.

• Assures that patient care follows the established Plan of Care, the organization’s guidelines, meets professional standards, community needs and is in compliance with Federal and State guidelines.
• Under the supervision of a Physical Therapist, identifies and develops a comprehensive plan of care; Develops and implements individualized therapy programs designed to restore, reinforce and enhance performance.
• Identify need for and makes referral to other disciplines to address the comprehensive needs of the patient under the supervision of a Physical Therapist.
