Join Evolve Therapy Services, LLC as a Physical Therapist Assistant (PTA) at our partner facility in Lawton, OK.

Job Position: In-house Skilled Nursing Facility

Job Type: Full Time, Part Time, or PRN

Job Purpose: Assist in planning, organizing, developing, and directing Physical Therapy services to ensure high-quality resident care, following applicable standards and regulations.

Job Responsibilities:

• Provide physical therapy services under the supervision of a Physical Therapist.
• Conduct resident screenings and evaluations.
• Assist in developing and implementing treatment plans.
• Conduct individual and group treatments focused on muscle strength, mobility, balance, gait, and modalities.
• Monitor resident responses and modify interventions as needed.
• Participate in resident and staff education.
• Complete documentation in NetHealth EMR systems.

Qualifications:

• Associates Degree in Physical Therapy from an accredited program.
• Active Physical Therapy Assistant license in the state of practice.
