ASHFORD CONNEX, LLC is seeking a passionate Physical Therapist Assistant to provide skilled home-based care in San Jose, CA. You will deliver PT interventions, develop individualized treatment plans, and collaborate with a multidisciplinary team to ensure coordinated care.

The role requires a CA PTA license, fingerprint clearance, CPR/ACLS, TB test and vaccines, and a valid driver’s license. This is a hands-on home care position in the San Jose area.

Qualifications

• Current Physical Therapist Assistant license in California.
• Valid CA Fingerprint Clearance Card.
• BLS (CPR) / ACLS certification.
• Recent TB test result and Flu Shot within the past year.
• Immunization records including Covid-19, Hep B, MMR, TDaP, Varicella.

• Create and execute individualized treatment plans tailored to each patient.

Required Qualifications

• Recent Negative TB test result and Flu Shot within the past year.
• Comprehensive Immunization/Vaccination Records, including Covid19, Hep B, MMR, TDaP, and Varicella.
• Valid Driver's License along with auto-insurance.
