Clear Concepts, Inc. is seeking a dedicated Physical Therapist Assistant to provide in-home PT services in San Jose, CA. You will assess patients, implement treatment plans, and support mobility and function in the home setting.

The role requires a California PTA license, fingerprint clearance, CPR/ACLS, TB testing, flu vaccination, and a full vaccination record. A valid driver's license with auto-insurance is also required.

Formación

• Current Physical Therapist Assistant license in California.
• Valid CA Fingerprint Clearance Card.
• Up-to-date BLS (CPR) and ACLS certification.
• Recent TB test result and Flu Shot within the past year.
• Comprehensive Immunization/Vaccination Records, including Covid19, Hep B, MMR, TDaP, and Varicella.
• Valid Driver's License along with auto-insurance.

Responsabilidades

• Delivering skilled physical therapy interventions, including exercises and stretches, aimed at enhancing patients' mobility and function.
• Creating and executing individualized treatment plans tailored to each patient's specific needs and goals.

• Comprehensive Immunization/Vaccination Records, including Covid19, Hep B, MMR, TDaP, and Varicella.
• Valid Driver's License along with auto-insurance.
