Nexushm is seeking a Physical Therapist Assistant to work under a licensed Physical Therapist, delivering rehab services to residents to improve mobility, strength, balance, and independence. You will implement treatment plans and monitor progress to support recovery and safety while contributing to quality of life.

Ideal candidates hold an Illinois PTA license with preferred SNF/long-term care experience and familiarity with PCC.

• SNF/Long term care experience preferred.
• Knowledge of PCC preferred.

• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function.
• Provides skilled physical therapy services under the supervision of a Physical Therapist.
• Relays information and referrals to the Physical Therapist on requests for service.
• Implements and adjusts the patient’s plan of treatment under PT supervision.
• Measures and records patient motor function, strength, and muscle performance.

Physical Therapist Assistant

Position Overview:

Work under the direction and supervision of a licensed Physical Therapist to provide rehabilitation services that help residents improve mobility, strength, balance, and functional independence. The PTA implements treatment plans designed to support residents’ recovery, safety, and overall quality of life.

Key Responsibilities:

• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.

• Provides skilled physical therapy services/interventions in accordance with physician orders under the supervision of a Physical Therapist.

• Responds to requests for service by relaying information and referrals to Physical Therapist.

• Implements and modifies the patient’s physical therapy plan of treatment with PT supervision.

• Measures & records patient’s motor function, strength, and muscle performance.

• SNF/Long term care experience preferred.
