Job summary

Trilogy Health Services, LLC is seeking a Physical Therapy Assistant to support patient care at Waunakee Valley Senior Living in Wisconsin. The role involves implementing treatment plans, documenting progress, and guiding patients and families on exercises and safety at home.

You will collaborate with Physical Therapists and an interdisciplinary team to ensure comprehensive care. Associate degree and PTA licensure are required; CPR is preferred. Travel is minimal.

Qualifications

• Must have Associate Degree in Physical Therapy Assistance.
• 0-1 years of experience preferred.
• Must hold and maintain a current PTA licensure or certification; CPR certification preferred.

Responsibilities

• Implements treatment plans designed by Physical Therapists, including exercises and modalities.
• Observes patient responses and documents progress for the supervising PT.
• Maintains up-to-date documentation of patient responses and progress and reports findings to the supervising PT.
• Provides patients and families with instructions on exercises, safety precautions, and home progress.
• Supports Physical Therapists in evaluating patient needs and progress.
• Collaborates with interdisciplinary team to ensure comprehensive patient care.
• Performs other duties as assigned.

Position Overview

• Implements treatment plans designed by Physical Therapists, including therapeutic exercises, manual therapy techniques, and modalities (e.g., heat, cold, electrical stimulation).
• Observes patient responses to therapy, document their progress, and report findings and any concerns to the supervising Physical Therapist.
• Observes and maintain up-to-date documentation related to patients’ responses to therapy, document their progress, and report findings to the supervising Physical Therapist.
• Provides instructions to patients and their families on exercise techniques, safety precautions, and strategies to support continued progress at home.
• Supports Physical Therapists in evaluating patient needs and progress, providing additional observations and insights.
• Collaborates with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.

Education: Associate Degree

Experience: 0-1 years

Licenses and Certifications

Must have and maintain a current, valid state PTA licensure or certification

Current valid CPR certification, preferred

Travel: Yes. Minimally, as required (Typically less than 10%)
