Job summary

HealthPro Heritage in Tomball, TX is seeking a Physical Therapist Assistant (PRN) to support a senior living and community-based therapy team. You will assist Physical Therapists by providing direct patient care, implementing treatment plans, and documenting progress in the EHR.

This role offers flexible scheduling, collaboration with a multidisciplinary team, and opportunities to contribute to resident outcomes across nursing facilities, retirement communities, hospitals, home care settings.

Qualifications

• Associate's degree in Physical Therapist Assisting from an accredited program is required.
• Valid state licensure as a Physical Therapist Assistant or license eligible is required.
• Strong assessment, communication, and documentation abilities with EHR proficiency.

Responsibilities

• Assist in implementing treatment plans designed by Physical Therapists and provide direct patient care.
• Monitor and document patient progress, report observations to the Physical Therapist, and educate patients and families on protocols.
• Accurately document treatments, progress, and responses in compliance with facility policies and standards.
• Collaborate with Physical Therapists and other healthcare professionals to ensure coordinated patient care.

Qualifications

• Education: Associate's degree in Physical Therapist Assisting from an accredited program.
• Licensure: Valid state licensure as a Physical Therapist Assistant, or license eligible
• Skills: Strong assessment, diagnostic, and therapeutic abilities with excellent communication and interpersonal skills. Proficient in using diagnostic tools, therapy equipment, relevant software, and electronic health record systems, with a proven ability to work collaboratively in a multidisciplinary team.
