An established industry player is seeking a dedicated Physical Therapist Assistant to join their team. The Home Health PRN Physical Therapist Assistant (PTA) performs treatment, provides exercise instruction, and patient education aimed at improving or enhancing the patient's well-being, following the established plan of care.
Under the supervision of a Physical Therapist, the PTA is responsible for:
Following the plan of care, the PTA instructs and aids patients in evidence-based treatment within their scope and according to state laws. They observe, record, and report patient responses and condition changes to the supervising PT, nurse, and/or physician, and coordinate care with the healthcare team. The PTA also educates patients, families, caregivers, and other team members within their scope.
Visits are conducted across the service area and nearby agencies as needed. After each visit, a progress note is completed and submitted as per policy. Flexibility in scheduling is required, including participation in weekend rotations and observed holidays to meet patient needs. The PTA participates in staff conferences and committees and adheres to LHC Therapy Standards of Practice. All other duties as assigned are also part of the role.
