Job summary

HealthPro Heritage in Hudson, OH is seeking a Licensed Physical Therapist Assistant (LPTA) for a PRN role at Crown Center at Laurel Lake. Join a continuum-of-care campus with diverse residents and an interdisciplinary team to deliver specialized therapy services.

You will implement treatment plans, document progress, and collaborate with Physical Therapists and staff to support resident outcomes. Eligible candidates must hold an associate degree and state licensure or licensure eligibility;

Qualifications

• Associate degree in Physical Therapist Assisting required.
• Valid state licensure as a Physical Therapist Assistant, or license eligible.
• Excellent communication and interpersonal skills and ability to collaborate in a multidisciplinary team.

Responsibilities

• Assist in implementing treatment plans designed by Physical Therapists; provide direct patient care including exercises, manual therapy, and modalities.
• Monitor and document patient progress, report observations to the Physical Therapist, and educate patients and families on treatment protocols.
• Accurately document patient treatments, progress, and responses in compliance with facility policies and regulatory standards.
• Work closely with Physical Therapists, other healthcare professionals, and support staff to ensure coordinated patient care and outcomes.

Need: PRN

Location: Crown Center at Laurel Lake in Hudson, OH

Setting: CCRC campus with 75 skilled beds, 70-ish AL beds, and 300 IL Beds.

Rate Range: $32-$39/hr (based on years of experience)

Qualifications

• Education: Associate’s degree in Physical Therapist Assisting from an accredited program.
• Licensure: Valid state licensure as a Physical Therapist Assistant, or license eligible
• Skills: Strong assessment, diagnostic, and therapeutic abilities with excellent communication and interpersonal skills. Proficient in using diagnostic tools, therapy equipment, relevant software, and electronic health record systems, with a proven ability to work collaboratively in a multidisciplinary team.
