Job summary

HealthPro Heritage in Sunrise at Parma, OH is seeking a Physical Therapist Assistant (PTA) for PRN coverage in assisted living and memory care. You will support therapy sessions under Physical Therapists, implement treatment plans, and help residents improve mobility and function in a home-like setting.

Qualifications

• Bachelor’s level description or higher is not listed; PTA-focused educational requirement is an Associate’s degree.
• Strong assessment, diagnostic and therapeutic abilities with excellent communication and collaboration.
• Proficiency with therapy equipment and relevant software and multidisciplinary teamwork.

Responsibilities

• Assist in implementing treatment plans designed by Physical Therapists and provide direct patient care including exercises, manual therapy and modalities.
• Monitor and document patient progress; report observations; educate patients and families on protocols and self-care techniques.
• Accurately document treatments, progress and responses per facility policies and regulatory standards.
• Collaborate with Physical Therapists, healthcare professionals and support staff to ensure coordinated care and outcomes.

Need: PRN - PTO illness/planned vacation

Location: Sunrise at Parma in Cleveland, OH

Setting: Assisted Living and memory care - award-winning!

Rate Range: $35-$37/hr (based on years of experience).

Qualifications

• Education: Associate’s degree in Physical Therapist Assisting from an accredited program.
• Licensure: Valid state licensure as a Physical Therapist Assistant, or license eligible
• Skills: Strong assessment, diagnostic, and therapeutic abilities with excellent communication and interpersonal skills. Proficient in using diagnostic tools, therapy equipment, relevant software, and electronic health record systems, with a proven ability to work collaboratively in a multidisciplinary team.
