Job summary

Continuum Therapy Partners is seeking a Physical Therapist Assistant to join our team in Galion, Ohio. The role supports a therapist-led approach across a national footprint and emphasizes patient-focused care and education under the supervision of a licensed Physical Therapist.

Responsibilities include implementing individualized plans, documenting progress, and educating patients on home exercises and assistive devices. Position is PRN with daytime shifts at Mill Creek and Galion locations.

Qualifications

• Associate degree required for PTA.
• Current, unencumbered state PTA license.
• 1 year of PTA experience preferred.
• Implements individualized treatment plans under supervision of a PT.
• Documents patient progress and notifies supervisor of changes.
• Educates patients on home exercise programs and use of assistive devices.

Physical Therapist Assistant Requirements & Responsibilities

• Associate Degree in Physical Therapist Assistant Program required
• Current, unencumbered state Physical Therapist Assistant License
• 1 year of previous experience as a Physical Therapist Assistant preferred
• Implements individualized treatment plans under the supervision of a Physical Therapist
• Documents patient progress and reports changes in patient status to the supervising PT
