Job summary

Concierge Home Care is seeking a Home Health Physical Therapist Assistant (PTA) to deliver one-on-one care in patients' homes in Bradenton, FL. Work with a flexible schedule and autonomy, optimizing visit timing to maximize outcomes while following the plan of care.

You’ll use AI-assisted charting and WellSky/Kinnser to streamline documentation and coordinate with the supervising PT. The role offers benefits including health, dental and vision insurance options, PTO starting at 3 weeks, a 401(k)

Qualifications

• Active, unencumbered Florida PTA license.
• Valid driver’s license and reliable transportation.
• Home health experience preferred.

Responsibilities

• Collaborate with patients, caregivers and interdisciplinary teams.
