Job summary

Concierge Home Care is seeking a Home Health PTA to deliver one‑on‑one therapy in patients’ homes across Fort Walton Beach and Okaloosa County. You’ll enhance mobility, strength and independence while following the treating PT’s plan of care.

Flexible schedules, autonomy, and strong earning potential with bonuses are offered. An active Florida PTA license and reliable transportation are required; home health experience is preferred but not required.

Responsibilities

• Provide hands-on therapeutic care following the plan of care established by the Physical Therapist.
• Assist patients in improving mobility, strength, and independence.
• Collaborate with patients, caregivers, and interdisciplinary teams.
• Monitor patient progress and communicate updates to supervising PT.
• Complete documentation using streamlined, AI-assisted tools.

Your Role as a Home Health Physical Therapist Assistant
