Job summary

Kern River Transitional Care is seeking a compassionate Physical Therapist Assistant (PTA) to join our rehabilitation team on a PRN basis. The PTA will work under the supervision of a licensed Physical Therapist to help residents improve mobility and restore function.

We offer a supportive team, continuing training opportunities, and a resident-centered environment within a skilled nursing setting. California PTA licensure and CPR certification are required.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current California PTA license in good standing.
• Current CPR certification.
• Experience in skilled nursing or long-term care preferred.
• Strong communication, documentation and interpersonal skills.
• Compassionate, dependable and committed to resident care.

Responsibilities

• Provide physical therapy treatments according to the plan of care established by the Physical Therapist.
• Assist residents with therapeutic exercises, mobility training, transfers, balance and gait activities.
• Observe and document residents’ progress, responses and changes in condition.
• Communicate resident updates and concerns to the supervising Physical Therapist.
• Educate residents, families and caregivers on exercises and safety techniques.
• Maintain accurate and timely clinical documentation.
• Follow facility policies and applicable state and federal regulations.
• Collaborate with nursing, rehabilitation and interdisciplinary team members.
• Promote a safe, resident-centered environment.

Kern River Transitional Care

Pay Range: $42.00–$45.00 per hour, based on experience

Kern River Transitional Care is seeking a compassionate and motivated Physical Therapist Assistant (PTA) to join our rehabilitation team on a PRN basis. The PTA will work under the direction of a licensed Physical Therapist to help residents improve mobility, restore function, and achieve their highest level of independence.

Key Responsibilities

• Provide physical therapy treatments according to the plan of care established by the Physical Therapist
• Assist residents with therapeutic exercises, mobility training, transfers, balance, and gait activities
• Observe and document residents’ progress, responses, and changes in condition
• Communicate resident updates and concerns to the supervising Physical Therapist
• Educate residents, families, and caregivers on exercises and safety techniques
• Maintain accurate and timely clinical documentation
• Follow all facility policies and applicable state and federal regulations
