Job Profile Summary

This role focuses on assessing the needs of patients with physical, mental, or emotional disabilities and delivering therapy services to improve their developmental, physical, or cognitive impairments. It involves providing services that help restore function, improve mobility, relieve pain, and prevent or limit permanent physical disabilities. The role includes examining patients' medical histories, evaluating their physical abilities, developing treatment plans, and performing specialized knowledge or skills gained through vocational education, training, and experience. This position primarily applies to patient care and scientific/laboratory jobs, working under moderate supervision and requiring interpretation or deviation from standard procedures when necessary.

Job Overview

This position collaborates with a licensed Physical Therapist to provide rehabilitative care in various settings such as inpatient, transitional care, and outpatient facilities. The goal is to help patients regain movement and mobility due to illness, injury, aging, or other disorders. The role may also involve helping patients manage pain, meet fitness goals, or prevent injuries through methods like heat or ice therapy, stretching, massages, and other activities.

Job Description
Minimum Qualifications:
  • Associate’s Degree in Physical Therapy
  • Physical Therapy Assistant license
  • Basic Life Support (BLS) certification
  • One (1) year of related experience
  • A current driver’s license may be required depending on the role
Preferred Qualifications:
  • Three (3) years of community health experience
Duties and Responsibilities:

The following are general duties; other tasks may be assigned:

  • Provide care and services as planned, delegated, and supervised by the Physical Therapist, according to the care plan.
  • Document services in compliance with agency policies and standards.
  • Prepare visit notes with therapy provided, patient responses, teaching, objective data, and communicate with the licensed therapist about the patient’s condition and response.
  • Participate in in-service education, staff meetings, record reviews, and policy reviews as directed.
  • Schedule and conduct therapy sessions at times beneficial to patients.
  • Complete all required documentation timely and accurately.
  • Maintain a safe, therapeutic environment for patients.
  • Report and review changes in patient conditions with the registered therapist.
  • Work effectively with patients, families, co-workers, and community resources.
  • Educate patients and families according to the care plan.
  • Coordinate services and participate in interdisciplinary team (IDT) meetings.
  • Comply with policies, including not canceling confirmed assignments without approval, providing adequate schedule change notifications, and timely completion of time sheets.
  • Report concerns related to corporate compliance to the CEO or Chief Compliance Officer.
  • Ensure compliance with regulatory agencies and internal policies, and practice confidentiality per HIPAA/HITECH guidelines.
Physical Requirements:
  • Prolonged standing, walking, and transferring patients.
  • Ability to lift and/or move up to 25 pounds regularly and up to 100 pounds occasionally.
  • Manual dexterity for testing and manual treatments.
  • Visual acuity for computer use and reports.
  • Effective communication skills for internal and external exchanges.

  • Ability to function in busy situations.
  • Responsible and reliable.
  • Proficiency in English reading, writing, and communication.
  • Ability to assess the needs of patients across different age groups for appropriate care.
