Encore at Foulk in Delaware is seeking a Full-Time Physical Therapist Assistant to support patients across Parkside, Foulk, and Wilmington under supervision of a licensed PT. The role focuses on delivering prescribed therapies to improve independence and function.

The ideal candidate will have PTA program graduation, DE licensure, CPR certification, and strong communication skills. A $2,500 sign-on bonus is offered with eligibility after one year.

Qualifications

  • Graduate of an accredited PTA therapy program.
  • Current DE state licensure.
  • CPR certification.
  • Excellent verbal, written, and interpersonal communication skills.
  • Results-oriented with strong critical thinking and problem-solving skills.
  • Proficient with therapy documentation (NetHealth).
  • Must pass a medical and physical examination.

Responsibilities

  • Attends patient care and staffing conferences as a representative of physical therapy.
  • Ensures efficient delivery of rehab services and timely documentation.
  • Collaborates with rehabilitation team to implement patient programs.
  • Assists PT during tests, evaluations, and procedures.
  • Instructs patients and families in therapy techniques under PT supervision.
  • Maintains documentation for all patients treated.
  • Cleans and maintains equipment and reports repairs as needed.
  • Implements individualized treatment plans with PT oversight.
  • Provides PT treatments including therapeutic exercise, manual therapy, and modalities.
  • Organizes treatment groups and performs related duties.

Encore at Foulk is currently hiring for a Full-Time Physical Therapist Assistant.

$2,500 SIGN ON BONUS

The Physical Therapist Assistant will work across Encores 3 communities:Parkside, Foulk, and Wilmington. The Physical Therapist Assistant provides physical therapy treatments to patients to facilitate increased independence and functioning under the supervision of a Licensed Physical Therapist.

RESPONSIBILITIES/ACCOUNTABILITIES:
  • Attends and contributes to patient care, staffing conferences, and other required meetings as a representative of physical therapy, under the direction of the physical therapist.

    Responsible for the effective and efficient delivery of rehab services; adherence to an established plan of care, department, and patient(s)' schedule, and timely documentation of services.

    Works with other members of the rehabilitation team to implement programs and activities consistent with the needs and capabilities of each patient.

    Acts as an assistant to the Physical Therapist when the Physical Therapist is performing tests, evaluations, and complex procedures.

    Instruct patients, families, and other caregivers in the skills and techniques of the physical therapy treatment program under the supervision of the Physical Therapist.

    Maintains appropriate and timely documentation for all patients treated.

    Cleans, maintains, identifies, and reports the need for repair of equipment, as necessary.

    Implements a patient's individualized treatment plan as established by the primary Physical Therapist.

    Provides individualized physical therapy treatments including but not limited to:

    Therapeutic exercise including aerobic capacity conditioning, balance coordination, agility training, body mechanics and postural stabilization, flexibility exercises, gait, and locomotion training, neuromotor development training, relaxation, strength, power, and endurance training. Functional training

    Manual therapy

    Airway clearance techniques

    Integumentary repair and protection

    Use of electrotherapeutic, physical agent, and mechanical modalities

    Organizes and facilitates treatment groups in collaboration with the physical therapist.

    Performs other related duties as required.

QUALIFICATIONS:

Graduate of an accredited Physical Therapist Assistant (PTA) therapy program.

Current DE state licensure.

CPR Certified

Excellent verbal, written, and interpersonal communication skills.

Results-oriented with strong critical thinking and problem-solving skills.

Proficient with therapy documentation (NetHealth).

Must pass a medical and physical examination based on job qualifications, requirements, and duties/responsibilities.

PHYSICAL DEMANDS:

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand, walk, use hands to touch, handle or feel, and reach with hands and arms.

Must be able to lift 501bs- 1001bs.

Use of proper body mechanics and techniques for bearing weight.

Must be able to climb stairs, stoop, and reach when performing daily work.

Ability to assist and perfonn patient transfers, (including but not limited to: stand pivot, two-person, sliding board, Hoyer lift, non-weight bearing) safely and effectively with or without mechanical devices.

Specific vision abilities required by this job include close vision, distance vision, color vision, and or other duties.


The $2,500sign-onbonuswill be paid in installments. Continued employment with the company for a minimum ofone (1) year from the start date is required. If employment ends prior to theone-year anniversary, any unpaid portion of thebonuswill be forfeited
