Job summary

Spring Creek Healthcare Center in Port Reading, NJ seeks a Physical Therapist Assistant (PTA) to join our rehabilitation team and support resident recovery under the supervision of a Physical Therapist.

The PTA will implement treatment plans, assist with assessments, document progress in EMRs, educate residents and families, and collaborate with an interdisciplinary rehab staff to optimize mobility and independence.

Qualifications

• State licensure as a Physical Therapy Assistant.
• EMR and MS Office applications.
• CPR/BLS certification.
• Knowledge of Section GG data collection and documentation requirements.

Responsibilities

• Assist in Assessments: carry out treatment plans and collect objective data for Section GG.
• Treatment Support: implement plans including exercises, modalities, and mobility training.
• Monitoring: observe residents’ performance and report changes to the supervising PT.
• Documentation: maintain accurate progress notes and EMR entries (Net Health/PCC).
• Education: educate residents, families, and care staff on safe mobility techniques.
• Team Collaboration: work with the interdisciplinary rehab staff under the PT guidance.
