Job summary

Addus HomeCare in Santa Fe, NM is seeking a dedicated Physical Therapist Assistant to join our growing home health care team. This full-time position offers Monday through Friday 8am to 5pm, with responsibilities delivering direct physical therapy under a licensed PT and per the plan of care.

You’ll assist patients with exercises, gait training, and functional activities, document progress, and coordinate with supervising clinicians.

Qualifications

• Graduate of an accredited physical therapy assistant program with an associate degree approved by the APTA.
• Current licensure as a Physical Therapist Assistant in the state of practice.
• CPR certification valid and current.

Responsibilities

• Provides direct physical therapy according to directions of a qualified physical therapist and the physician's plan of care.
• Treats patients to relieve pain, develop or restore function, and maintain maximum performance.
• Instructs and assists patients in exercises, gait training, and functional activities.
• Administers non-complex modalities and documents treatments in the clinical record.
• Observes patient progress and reports findings to the supervising Physical Therapist.
• Prepare clinical notes and progress reports for review in the patient record.
• Instructs patients in care and use of assistive devices as appropriate.
