Job summary

Apex Home Health, Hospice & Home Care is seeking a licensed Physical Therapy Assistant to provide patient-centered therapy in patients' homes under the supervision of a licensed PT. You will deliver therapeutic exercises, gait training, and functional mobility interventions to support independence and safe ambulation, while collaborating with an interdisciplinary team.

Strong communication with patients and caregivers, accurate EMR documentation, and the ability to work autonomously in home

Qualifications

• Current licensure/credential as a Physical Therapist Assistant (PTA).
• Strong clinical skills in therapeutic exercise, gait training, and functional mobility.
• Excellent communication and patient/caregiver education abilities.
• Ability to work independently in home settings while following PT supervision.
• Accuracy and professionalism in EMR documentation and teamwork.
