You will deliver person-centered therapy under the direction of a Physical Therapist, applying individualized care plans and documenting progress with our team.

Qualifications

• Graduate from accredited Physical Therapist Assistant program.
• Current state licensure -or- in the process of obtaining appropriate state licensure.
• Experience in Post-Acute care preferred.

Responsibilities

• Apply a transformative approach to therapy by implementing personalized care plans under the direction and supervision of a Physical Therapist.
• Utilize AgeRight Therapy's evidence-based clinical programs and pathways to ensure optimal patient outcomes.
• Be a Champion of the Journey by maximizing function in physical, emotional, social, and cognitive realms for patients, families, co-workers, and self.
