Diamond Healthcare is seeking a dedicated Physical Therapist Assistant to join our team in a Skilled Nursing Facility. You will work closely with licensed physical therapists to deliver high-quality care, developing and implementing individualized plans to help patients regain mobility and improve quality of life.

In this in-person role, you will educate patients and families, monitor progress, and collaborate with a broader healthcare team.

Qualifications

• Previous experience or coursework in physical therapy is preferred.
• Familiarity with pediatrics, orthopedics, outpatient settings, or home health is a plus.
• Knowledge of medical terminology and physiology is essential for effective communication within the healthcare team.
• Experience in developing care plans tailored to individual patient needs is highly valued.

Responsibilities

• Assist physical therapists in developing and implementing individualized treatment plans based on patient assessments.
• Provide therapeutic interventions to patients, including exercises and modalities, under the supervision of a licensed physical therapist.
• Monitor patient progress and document treatment outcomes accurately in compliance with clinic protocols.
• Educate patients and their families on care plans, home exercises, and injury prevention strategies.
• Maintain a clean and organized treatment area, ensuring all equipment is sanitized and ready for use.
• Collaborate with healthcare professionals to ensure comprehensive patient care.
• Stay updated on best practices in physical therapy and participate in ongoing professional development.
