Job summary

Crossroads Home Health and Hospice is seeking a Physical Therapy Assistant to provide skilled PT services under the supervision of a licensed physical therapist. This role emphasizes patient-centered care in accordance with the individualized plan of care and professional practice standards.

The PTA will perform therapeutic exercises, gait training, and assistive device instruction, document clinical notes, and participate in interdisciplinary teams and inservice education.

Qualifications

• Graduate of an accredited PTA program approved by the American Physical Therapy Association (APTA).
• Meets personnel qualifications stated in 418.114(b)(8).
• Current certification or license as required by state law.
• Possesses and maintains current CPR certification.
• Must be a licensed driver with an automobile that is insured and in good working order.

Responsibilities

• Provides direct physical therapy under the direction of a licensed physical therapist and within the plan of care.
• Treats patients to relieve pain, restore function, and maintain maximum performance.
• Aids patients in active and passive therapeutic exercises, gait and functional training, and transfer activities.
• Utilizes therapeutic modalities including heat, cold, water, light, electricity, ultrasound, and massage.
• Observes and reports patient responses and changes to the physical therapist and Organization staff.
• Instructs patients in use of wheelchairs, braces, crutches, canes, and prosthetic/orthotic devices.
• Maintains clinical documentation and notes within prescribed timeframes.
• Participates in interdisciplinary group and Organization Inservice Education Program.
• Participates in quality assessment and performance improvement activities.
• Performs other duties as assigned.

Job Description Summary

The physical therapy assistant, contract or employed, provides physical therapy services under the direction of the physical therapist and according to the individualized plan of care.

Essential Job Functions/Responsibilities

• Provides direct physical therapy according to directions of a qualified physical therapist and in accordance with the plan of care and within the scope of the Physical Therapy Practice Act.
• Treats patient to relieve pain, develop or restore function, and maintain maximum performance.
• Directs and aids patients in active and passive therapeutic exercises, muscle reeducation, gait and functional training, transfer activities and prosthetic training.
• Utilizes physical agents, including, but not be limited to heat, cold, water, light, electricity, ultrasound and massage.
• Observes and reports to the physical therapist and the Organization personnel the patient’s reaction to treatment and any changes in patient’s condition.
• Instructs patients in care and use of wheelchairs, braces, crutches, canes, and prosthetic or orthotic devices.
• Maintains necessary records; clinical notes and conference notes, which will be incorporated into the patient’s clinical record in prescribed time frames.
• Participates in interdisciplinary group and Organization Inservice Education Program.
• Participates in the quality assessment performance improvement teams and activities.
• Performs other duties as assigned.

Position Qualifications

• Graduate of an accredited physical therapy assistant program with either an Associate’s degree in science or applied science that is approved by American Physical Therapy Association, Inc. Meets personnel qualifications stated in
