Job summary

Crossroads Home Health and Hospice is seeking a Physical Therapy Assistant to provide physical therapy services under the direction of a licensed physical therapist and according to the individualized plan of care.

The role requires graduation from an accredited PTA program, current CPR certification, and a valid driver’s license. You will contribute to patient progress, documentation, and participation in quality improvement activities.

Qualifications

• Graduate of an accredited PTA program approved by the American Physical Therapy Association.
• Current state license or certification as required by state law.
• CPR certification in effect.
• Valid driver’s license with insured vehicle.

Responsibilities

• Provide direct physical therapy under the direction of a licensed physical therapist.
• Treat patients to relieve pain, develop or restore function, and maintain performance.
• Lead and assist patients in therapeutic exercises and gait training.
• Utilize physical agents (heat, cold, water, light, electricity, ultrasound, massage) as directed.
• Observe and report patient responses to treatment.
• Instruct patients in use of mobility aids (wheelchairs, braces, crutches, canes, prosthetic/orthotic devices).
• Maintain patient records and clinical notes within prescribed time frames.
• Participate in interdisciplinary group and Organization Inservice Education Program.
• Participate in quality assessment and performance improvement teams and activities.
• Perform other duties as assigned.

Job Description Summary

The physical therapy assistant, contract or employed, provides physical therapy services under the direction of the physical therapist and according to the individualized plan of care.

Essential Job Functions/Responsibilities

• Provides direct physical therapy according to directions of a qualified physical therapist and in accordance with the plan of care and within the scope of the Physical Therapy Practice Act.
• Treats patient to relieve pain, develop or restore function, and maintain maximum performance.
• Directs and aids patients in active and passive therapeutic exercises, muscle reeducation, gait and functional training, transfer activities and prosthetic training.
• Utilizes physical agents, including, but not be limited to heat, cold, water, light, electricity, ultrasound and massage.
• Observes and reports to the physical therapist and the Organization personnel the patient’s reaction to treatment and any changes in patient’s condition.
• Instructs patients in care and use of wheelchairs, braces, crutches, canes, and prosthetic or orthotic devices.
• Maintains necessary records; clinical notes and conference notes, which will be incorporated into the patient’s clinical record in prescribed time frames.
• Participates in interdisciplinary group and Organization Inservice Education Program.
• Participates in the quality assessment performance improvement teams and activities.
• Performs other duties as assigned.

Position Qualifications

• Graduate of an accredited physical therapy assistant program with either an Associate’s degree in science or applied science that is approved by American Physical Therapy Association, Inc. Meets personnel qualifications stated in 418.114(b)(8).
• Current certification or license as required by state law.
• Possesses and maintains current CPR certification.
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
