Job summary

Consonus Healthcare is seeking a Physical Therapist Assistant to deliver patient-centered therapy at a nursing home setting. You will develop and implement individualized care plans, guided by evidence-based programs, to improve function and quality of life for senior patients.

Ideal candidates are graduates of an accredited PTA program, possess or be pursuing state licensure, and hold CPR certification. NPI is required upon hire; new grads are welcome to apply for growth within the team.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current state licensure - or in the process of obtaining licensure.
• CPR Certification (state/location dependent).
• National Provider Identifier (NPI) required upon hire.
• Excellent organizational skills to facilitate program development.
