AgeRight Therapy at Valley Hi Nursing Home & Rehabilitation in Woodstock, IL seeks a Physical Therapist Assistant to deliver person-centered care under the supervision of a Physical Therapist. You’ll follow evidence-based programs, tailor care plans, and help patients improve function and quality of life.

Qualifications

• Graduate from an accredited Physical Therapist Assistant program.
• Current state licensure or in the process of obtaining licensure.

Responsibilities

• Apply a transformative approach to therapy by implementing personalized care plans under supervision of a Physical Therapist.
• Utilize AgeRight Therapy’s evidence-based clinical programs to ensure optimal patient outcomes.
• Champion functional outcomes across physical, emotional, social, and cognitive domains for patients and families.

Minimum Qualifications

• Graduate from accredited Physical Therapist Assistant program
• Current state licensure -or- in the process of obtaining appropriate state licensure

Preferred Qualifications

• Experience in Post-Acute care preferred

Salary Range: $30 - $40/hr
