Position Summary
Assess, plan, organize, and participate in rehabilitative programs that improve mobility, relieve pain, increase strength, and improve or correct disabling conditions resulting from disease or injury.
As a Physical Therapist Assistant, You Will
- Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.
- Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.
- Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
- Apply concepts of infection control/universal precautions in performing patient care.
- Ensure continuity of quality patient care delivered with appropriate documentation.
- Provide effective communication to patient/family, team members, and other health care professionals.
- Promote agency philosophy and administrative policies.
- Maintain and complete all documentation within company-stated guidelines.
Requirements
- Successful completion of an accredited physical therapy program and passing the state licensure exam.
- Valid Physical Therapy license.
- Valid state driver’s license.
- 1 year of home health experience preferred.
- AHA CPR certification.
- Valid auto insurance.
- HomeCare HomeBase experience preferred.
Physical Demands
- Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.
- Must be able to lift, carry, pull, or push up to 100 lbs.
