Job summary

Best Home Health Providers Inc. is seeking a dedicated Physical Therapist Assistant to support patients in their homes across the Bay Area.

You will work with a licensed Physical Therapist to help patients regain mobility and return to activities they love, delivering hands-on therapy in the patient's home environment. The per diem role offers a rate of $95 per visit, with flexible scheduling and the potential to convert to full-time based on performance.

Qualifications

• Associate's degree from an APTA-accredited PTA program.
• Active PTA license from the California Physical Therapy Board.
• Current CPR certification with AHA.
• Strong communication and organizational skills.
• Valid driver's license and reliable insured vehicle.

Responsibilities

• Deliver hands-on physical therapy under the direction of a licensed Physical Therapist, following each patient's care plan.
• Guide patients through therapeutic exercises, gait and transfer training, and the use of assistive devices.
• Apply physical modalities including heat, cold, ultrasound, electrical stimulation, and massage to relieve pain and restore function.
• Monitor patient progress and communicate changes to the Physical Therapist and care team.
• Document care accurately and on time, and participate in team meetings and ongoing training.
• Perform other duties as required for patient services

As a Physical Therapist Assistant with Best Home Health Providers, you will work alongside a licensed Physical Therapist to help patients recover, regain mobility, and return to the activities they love — all from the comfort of their homes.

Please note that this role is per diem with a rate of $95 per visit.

• Apply physical modalities including heat, cold, ultrasound, electrical stimulation, and massage to relieve pain and restore function.
• Monitor patient progress and communicate changes to the Physical Therapist and care team.
• Document care accurately and on time, and participate in team meetings and ongoing training.
• Perform other duties as required for patient services
