Job summary

Best Home Health Providers, Inc. is seeking a Physical Therapist Assistant to support patients at home across the Bay Area and beyond. You will work with a licensed Physical Therapist to help patients regain mobility and return to daily activities, delivering hands-on care in patients’ homes.

This per diem role offers a $95 per visit rate, flexible scheduling, mileage reimbursement, and potential for full-time conversion based on performance. Travel to patient homes is required.

Qualifications

• Associate's degree from an APTA-accredited PTA program.
• Active PTA license from the California Physical Therapy Board.
• Current CPR certification with AHA.
• Strong communication and organizational skills.
• Valid driver's license and willingness to travel to patient homes.

Responsibilities

• Deliver hands-on physical therapy under the direction of a licensed Physical Therapist, following each patient's care plan
• Guide patients through therapeutic exercises, gait and transfer training, and the use of assistive devices such as wheelchairs, braces, crutches, canes, and prosthetics
• Apply physical modalities including heat, cold, ultrasound, electrical stimulation, and massage to relieve pain and restore function
• Monitor patient progress and communicate changes to the Physical Therapist and care team
• Document care accurately and on time, and participate in team meetings and ongoing training
• Perform other duties as required for patient services
