Qualifications

• Licensed PTA in Illinois is required.

Responsibilities

• Provide PTA services to residents in a SNF setting.
• Collaborate with therapists and nursing staff to develop and implement care plans.
• Document patient progress and ensure accurate treatment records in Net Health.
