Keystone Ridge Post Acute Nursing and Rehabilitation in Omaha, NE is seeking a full-time Physical Therapist Assistant (PTA) for in-house rehab. You will screen, evaluate, and deliver skilled therapy to geriatric patients, following regulatory standards.

New grads welcome; PTA license required. This role involves standing, walking, and assisting with wound care, modalities, and discharge summaries while supporting families and care teams.

Qualifications

• Licensed PTA with ability to provide skilled therapy under supervision.
• Excellent patient care and communication with patients and families.
• Ability to work in a team-based rehabilitation setting.

Responsibilities

• Screen and evaluate patients; develop care plans in line with regulatory standards.
• Deliver skilled therapeutic interventions and wound care as needed.
• Assist with discharge summaries and patient education for caregivers and families.
• Perform procedures including exercises, modalities, and use of assistive devices.
• Provide counseling to patients and families on physical disorders and recovery plans.
• Generate discharge summaries in compliance with clinical requirements.

PHYSICAL THERAPIST ASSISTANT – FULL TIME
KEYSTONE RIDGE POST ACUTE - OMAHA, NE

Keystone Ridge Post Acute Nursing and Rehabilitation in Omaha, NE, is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a dedicated Full Time Physical Therapist Assistant (PTA) for our in-house rehab program!

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• The role involves regular standing and walking throughout the facility and occasional lifting, transferring, and repositioning of patients and equipment. Physical demands vary throughout the day based on patient care needs.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

Qualifications

• Physical Therapist Assistant license is required.
• Open to Physical Therapist Assistants at all experience levels. New Grads are welcome!
