Job description

Overview

Provides physical therapy interventions to designated patients under the supervision of a Physical Therapist ("PT"). Assists in monitoring patient progress through observations and follows up with the supervising therapist. Works under PT supervision as required under NY State Education law. Follows established physical therapy plan of care, including interventions and patient/caregiver education. Modifies specific treatment procedures in accordance with PT plan of care and PTA scope of practice.

Responsibilities include:

  1. Documenting all care provided and maintaining records per VNS Health policy and standards.
  2. Assisting in monitoring patient progress and following up with the Supervising Therapist in accordance with VNSNY policy.
  3. Complying with the scope of practice for PTA, including co-visits with the PT on initial visits and keeping the PT informed. The PT will re-assess the patient every 6 visits or 30 days, whichever comes first.
  4. Managing assigned caseload and providing services as directed per PT plan of care.
  5. Participating in VNS Health meetings, including interdisciplinary team meetings, in-service and training programs, rehab services meetings, and caseload reviews.
  6. Traveling to patients' homes and other facilities with varying environments using approved transportation to deliver direct care.
  7. Assisting patients with therapeutic interventions and activities of daily living, including positioning, transferring, and lifting, and instructing family members and paraprofessionals as needed.
  8. Using and assisting with durable medical equipment as part of the plan of care.
  9. Providing care in accordance with agency policies, practices, procedures, and standards, which may require various physical activities.
  10. Transporting and utilizing technology devices and supplies using designated carrying cases.
  11. Participating in special projects and performing other duties as assigned.
    Qualifications

    Licenses and Certifications:

    • New York State Certification as a Physical Therapist Assistant required.
    • Valid driver's license may be required based on operational needs.

    Education:

    • Associate's Degree as a Physical Therapist Assistant from an approved NY State program or equivalent, required.

    Work Experience:

    • Minimum of two years of clinical patient care experience, required.
    • Bilingual skills and basic PC skills are preferred, depending on operational needs.
    Compensation

    $30.71 - $38.41 Hourly
