INNOVATE REHAB AND WELLNESS in Milan, MO is seeking a Licensed Physical Therapy Assistant (PTA) for a Skilled Nursing Facility. You will work Monday–Friday under the supervision of a licensed Physical Therapist, delivering patient-centered care to improve mobility and function.

The role emphasizes hands-on treatment, treatment plan implementation, and collaboration with the care team. Requires an associate degree and PTA license; CPR may be preferred. Competitive hourly pay based on experience.

Qualifications

• Associate degree from an accredited PTA program.
• Valid state PTA license.
• Rehab/clinical experience preferred (new grads welcome).
• Strong communication and interpersonal skills.
• CPR certification preferred.

Responsibilities

• Implement patient-specific treatment plans created by the PT.
• Lead patients through exercises, mobility training, and rehab activities.
• Educate patients on safe techniques and assistive devices.
• Monitor and document patient progress; report changes to the PT.
• Maintain accurate, compliant therapy records.
• Collaborate with the care team for coordinated treatment.
• Uphold ethical, professional standards in all interactions.

Position Summary

As a Physical Therapy Assistant, you will:

• Support and deliver individualized treatment plans
