Job description

Job Overview

This position works in collaboration with a licensed Physical Therapist to provide rehabilitative care to patients in various settings such as inpatient, transitional care, and outpatient. The goal is to help patients regain movement and mobility due to illness, injury, aging, or other disorders. The role may also involve helping patients manage pain, meet fitness goals, or prevent injury through methods like heat or ice therapy, stretching, and massages.

Hours

Full-Time, Days 8-4:30 with required weekend rotation and holidays

Minimum Qualifications
  • Associate's Degree in Physical Therapy
  • Physical Therapy Assistant license
  • Basic Life Support (BLS) certification
  • One (1) year of related experience
  • A current driver's license may be required depending on the role
Preferred Qualifications
  • Three (3) years of community health experience
Duties and Responsibilities

The duties listed below describe the general nature of work and are not exhaustive. Other duties may be assigned.

  1. Provides care and services as planned, delegated, and supervised by the Physical Therapist, according to the plan of care.
  2. Documents services in compliance with agency policies and professional standards.
  3. Prepares visit notes detailing therapy provided, patient responses, teaching performed, and communicates patient status to the licensed therapist.
  4. Participates in in-service education, staff meetings, record reviews, and policy reviews when directed.
  5. Plans sessions promptly after notification, scheduling them at times beneficial to the patient.
  6. Completes all required documentation timely and according to standards.
  7. Maintains a safe, therapeutic environment for patients.
  8. Reports and reviews changes in the patient's condition with the registered therapist.
  9. Works effectively with patients, families, co-workers, and community resources.
  10. Educates patients and families per the care plan.
  11. Coordinates services to patients effectively.
  12. Participates in interdisciplinary team (IDT) meetings for care coordination.
  13. Follows policies regarding schedule changes, documentation, and reporting issues.
  14. Understands what to report regarding corporate compliance concerns.
  15. Ensures compliance with regulatory guidelines and internal policies.
  16. Practices confidentiality per agency and federal HIPAA/HITECH standards.
